I’ve just been working with a client recently who’s been treated for hypothyroidism for over 15 years.

Every year, she has been for her check-up, does the blood test, and got the same response:

“No change, keep taking the same dose. See you next year.”

That’s 15 years of being unwell.
15 years of medications for depression, pain, and fatigue.
15 years of being told nothing’s wrong—even as her health and confidence steadily crumbled.

It wasn’t until she looked at a private test - a full thyroid panel - that the missing piece of the puzzle finally clicked into place.

Her TSH? In range.
Her T4? Low end of normal.
But her free T3? It was off the bottom of the chart. 😮

👉 T4 (the standard medication) is only a storage hormone. It has to be converted into T3—the active form your cells actually use for energy.

👉 If your body isn’t converting T4 into T3 properly (which is shockingly common), you can feel absolutely awful - even if your TSH is “normal”.

And most NHS doctors aren’t allowed to test free T3.

Even if they request it, the labs often refuse.

So it’s not all the doctor’s fault - but the system is broken. And we’re the ones paying the price.

✅ Get a full thyroid panel that includes free T3 (not total T3)
✅ Ask for a print out of your results - don’t accept vague reassurance
✅ If necessary, invest in a private test (around £45 in the UK)
✅ Look at the T4 to T3 ratio. If FREE T4 looks okay but FREE T3 is low? That’s a conversion issue
